Applying for an Apartment
To apply for an apartment, you will need to submit a rental application and ACH form. If you don't meet our requirements of making three times the monthly rent and having three good years of rental history you will also need to submit a guarantor form. The ACH payment form included below is how we deduct the security deposit as well as future rental payments. Please complete it accordingly and attach either a voided check or screenshot from your mobile banking app of your account/routing numbers for verification. Please send in all necessary forms together in ONE email or it will not be excepted. All forms should be sent to our email: butlerplazaapartments@gmail.com

How to Download and Saving Forms:
To download or save a PDF/Word file, right click on the link, then from the popup menu select "Save target as" or "Save link as" and save to a folder on your computer. If the PDF file is open in your browser and you wish to save the file, simply click the disk icon in the Acrobat Reader toolbar or go to "File > Save..." and save to desired folder. Once the file is saved to your computer you can add text boxes to fill out the information to complete the form prior to emailing it to butlerplazaapartments@gmail.com

The Adobe® Acrobat® ReaderT is free, and freely distributable, software that lets you view, sign, and print Adobe Portable Document Format (PDF) files. If you are unable to view a PDF file with your web browser, you may download/save PDF files to your computer and open them with the free standalone Acrobat Reader. This can also be done on mobile.
